Which operating system are you creating the RecoverAssist media from?
What method did you use to create the RecoverAssist media? Was it build from a Microsoft issued installation disk/Windows iso file (which is the recommended method) or from the installed operating system?
If it wasn't built from an installation disk or Windows ISO file, then can you please try this as a live OS may have Windows Updates installed which can change the necessary system files and cause problems with the boot environment. Such an issue has been discussed at

Hopefully this helps get you over what you're currently seeing.
